{"id":82003,"date":"2021-11-18T09:12:29","date_gmt":"2021-11-18T09:12:29","guid":{"rendered":"https:\/\/theoutlier.co.za\/?p=82003"},"modified":"2025-11-17T17:43:49","modified_gmt":"2025-11-17T17:43:49","slug":"genome-research-enables-scientists-to-identify-coronavirus-mutations-and-is-vital-for-vaccine-development","status":"publish","type":"post","link":"https:\/\/outliereditor.co.za\/index.php\/2021\/11\/18\/genome-research-enables-scientists-to-identify-coronavirus-mutations-and-is-vital-for-vaccine-development\/","title":{"rendered":"In the race against new variants, Africa ups genome sequencing"},"content":{"rendered":"\n<p>In&nbsp;<a href=\"https:\/\/www.krisp.org.za\/publications.php?pubid=362\" target=\"_blank\" rel=\"noreferrer noopener\">October<\/a>&nbsp;2020, South African scientist&nbsp;<a href=\"https:\/\/www.afro.who.int\/news\/why-genomic-sequencing-crucial-covid-19-response\" target=\"_blank\" rel=\"noreferrer noopener\">Tulio de Oliveira<\/a>&nbsp;and his team, sequencing coronavirus samples at their lab in the eastern coastal city of Durban, were the first to detect the Beta variant, or B.1.351.<\/p>\n\n\n\n<p>\u201cWe immediately informed our Ministry of Health and our President&nbsp;<a href=\"https:\/\/standard-web-prod.brightsites.co.uk\/topic\/cyril-ramaphosa\" target=\"_blank\" rel=\"noreferrer noopener\">Cyril Ramaphosa<\/a>&nbsp;when we discovered the Beta variant in&nbsp;<a href=\"https:\/\/standard-web-prod.brightsites.co.uk\/topic\/south-africa\" target=\"_blank\" rel=\"noreferrer noopener\">South Africa<\/a>&nbsp;very early in our second wave (of&nbsp;<a href=\"https:\/\/standard-web-prod.brightsites.co.uk\/topic\/covid\" target=\"_blank\" rel=\"noreferrer noopener\">Covid<\/a>-19 infections),\u201d said&nbsp;<a href=\"https:\/\/www.afro.who.int\/news\/why-genomic-sequencing-crucial-covid-19-response\" target=\"_blank\" rel=\"noreferrer noopener\">Professor de Oliveira<\/a>, a bioinformatician who has become a familiar face on South African TV during the pandemic.<\/p>\n\n\n\n<p>Within a day, new legislation was drafted, putting the country into a stricter level of&nbsp;<a href=\"https:\/\/standard-web-prod.brightsites.co.uk\/topic\/lockdown\" target=\"_blank\" rel=\"noreferrer noopener\">lockdown<\/a>. \u201cIt took less than 48 hours of knowing about the new variant to do that,\u201d he said.<\/p>\n\n\n\n<p>De Oliveira is the director of KwaZulu-Natal Research Innovation and Sequencing Platform (KRISP) and the Centre for Epidemic Response and Innovation (CERI), part of a&nbsp;<a href=\"http:\/\/www.krisp.org.za\/ngs-sa\/ngs-sa_network_for_genomic_surveillance_south_africa\/\" target=\"_blank\" rel=\"noreferrer noopener\">network of scientific institutions<\/a>&nbsp;in South Africa established in June 2020 to ensure the public health system had access to the best possible scientific data about the SARS-CoV-2 virus.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"682\" src=\"https:\/\/editor.mediahack.co.za\/theoutlier\/wp-content\/uploads\/sites\/4\/2021\/11\/Tulio_portrait-2-1024x682.jpg\" alt=\"\" class=\"wp-image-82004\" srcset=\"https:\/\/outliereditor.co.za\/wp-content\/uploads\/2021\/11\/Tulio_portrait-2-1024x682.jpg 1024w, https:\/\/outliereditor.co.za\/wp-content\/uploads\/2021\/11\/Tulio_portrait-2-300x200.jpg 300w, https:\/\/outliereditor.co.za\/wp-content\/uploads\/2021\/11\/Tulio_portrait-2-768x512.jpg 768w, https:\/\/outliereditor.co.za\/wp-content\/uploads\/2021\/11\/Tulio_portrait-2-1536x1024.jpg 1536w, https:\/\/outliereditor.co.za\/wp-content\/uploads\/2021\/11\/Tulio_portrait-2.jpg 2000w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><figcaption>South African scientist&nbsp;<a href=\"https:\/\/www.afro.who.int\/news\/why-genomic-sequencing-crucial-covid-19-response\" target=\"_blank\" rel=\"noreferrer noopener\">Tulio de Oliveira<\/a> (Photo: Supplied)<\/figcaption><\/figure>\n\n\n\n<p>In the last week, De Oliveira\u2019s name has again been splashed across international media, this time because of the discovery of the Omicron variant. As part of the Network for Genomic Surveillance in South Africa, a&nbsp;collaboration of laboratories, scientists and academic institutions,&nbsp;&nbsp;De Oliveira reported that a new variant with a \u201cconstellation of mutations\u201d had been identified in samples sequenced in South Africa\u2019s Gauteng province, which is experiencing an uptick of Covid-19 cases.<\/p>\n\n\n\n<p>Omicron has already spread across the world.&nbsp;Scientists are urgently working to determine how transmissible and deadly it is, and whether it may be able to evade the protections of current vaccines. <\/p>\n\n\n\n<p>The work of De Oliveira and his colleagues has highlighted the critical importance of genomic sequencing, a process that identifies the genetic structure of the virus and enables scientists to identify mutations &#8211; allowing governments to make quick and informed public health decisions.<\/p>\n\n\n\n<p>In Africa, there are over 500 scientists working in 30 countries involved in genomic sequencing, according to the South Africa-based&nbsp;<a href=\"https:\/\/h3africa.org\/\" target=\"_blank\" rel=\"noreferrer noopener\">Human Heredity and Health in Africa (H3Africa)<\/a>&nbsp;initiative, which aims to create and support a pan-continental network of laboratories conducting research in genomic sciences.<\/p>\n\n\n\n<p>But the continent lags far behind the rest of the world when it comes to sequencing, said&nbsp;<a href=\"https:\/\/www.afro.who.int\/news\/covid-19-variants-prolong-africas-pandemic-wave\" target=\"_blank\" rel=\"noreferrer noopener\">Dr Matshidiso Moeti<\/a>, WHO Regional Director for Africa. \u201cOnly 1% of over 3 million Covid-19 sequences conducted worldwide occur in Africa,\u201d she said in September.<\/p>\n\n\n\n<p>\u201cCountries must step up surveillance because without genomic information, variants can spread undetected. You can\u2019t fix what you don\u2019t measure.\u201d<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Variants of concern<\/h2>\n\n\n\n<p>Like all viruses, SARS-CoV-2, mutates as it spreads. Some&nbsp;<a href=\"https:\/\/www.who.int\/en\/activities\/tracking-SARS-CoV-2-variants\/\" target=\"_blank\" rel=\"noreferrer noopener\">mutations<\/a>&nbsp;do not change the virus very much, but some may make it more contagious, or affect the performance of vaccines or how serious the disease is.<\/p>\n\n\n\n<p>When scientists note that a mutation has genetic changes that could predict faster transmissibility or increased virulence, the&nbsp;<a href=\"https:\/\/www.who.int\/en\/activities\/tracking-SARS-CoV-2-variants\/\" target=\"_blank\" rel=\"noreferrer noopener\">WHO<\/a>&nbsp;designates it a variant of interest.<\/p>\n\n\n\n<p>There are several variants of interest circulating around the globe, the&nbsp;<a href=\"https:\/\/www.who.int\/en\/activities\/tracking-SARS-CoV-2-variants\/\" target=\"_blank\" rel=\"noreferrer noopener\">WHO<\/a>&nbsp;has identified Lamda (first detected in Peru) and Mu (first detected in Colombia); while the Africa CDC is monitoring B.1.525 (first sampled in the UK) and A.23.1 (first detected in East Africa).<\/p>\n\n\n\n<p>A variant of interest becomes a variant of concern when the mutations make it more contagious or make the disease more transmissible and decrease the effectiveness of public health measures to control the virus \u2013 for instance, if the variant does not respond to vaccines.<\/p>\n\n\n\n<p>There are five designated\u00a0<a href=\"https:\/\/www.who.int\/en\/activities\/tracking-SARS-CoV-2-variants\/\" target=\"_blank\" rel=\"noreferrer noopener\">variants of concern<\/a>: Alpha, Beta, Gamma, Delta and Omicron. All five have been found in Africa, but\u00a0<a target=\"_blank\" href=\"http:\/\/go.redirectingat.com\/?id=45843X1573846&amp;url=https%3A%2F%2Fwww.newscientist.com%2Farticle%2F2279133-the-world-is-struggling-to-contain-the-delta-variant-of-coronavirus%2F&amp;sref=https:\/\/www.standard.co.uk\/node\/-1\" rel=\"noreferrer noopener\">Delta, or B.1.617.2<\/a>\u00a0is\u00a0<a target=\"_blank\" href=\"https:\/\/www.afro.who.int\/news\/covid-19-variants-prolong-africas-pandemic-wave#:~:text=To%20date%2C%20the%20dominant%20Delta,in%2044%20and%2039%20countries.\" rel=\"noreferrer noopener\">dominant<\/a>\u00a0at present.<\/p>\n\n\n\n<p>According to Dr Nicksy Gumede-Moeletsi, virologist with the WHO Regional Office for Africa, there is \u201cserious concern\u201d related to the Delta variant because, although responsive to vaccines, once it rears its head it seems to dominate over other variants.<\/p>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"1024\" src=\"https:\/\/editor.mediahack.co.za\/theoutlier\/wp-content\/uploads\/sites\/4\/2021\/12\/Variants-of-Concern-in-Africa-ES-December-2021-1024x1024.png\" alt=\"\" class=\"wp-image-82034\" srcset=\"https:\/\/outliereditor.co.za\/wp-content\/uploads\/2021\/12\/Variants-of-Concern-in-Africa-ES-December-2021-1024x1024.png 1024w, https:\/\/outliereditor.co.za\/wp-content\/uploads\/2021\/12\/Variants-of-Concern-in-Africa-ES-December-2021-300x300.png 300w, https:\/\/outliereditor.co.za\/wp-content\/uploads\/2021\/12\/Variants-of-Concern-in-Africa-ES-December-2021-150x150.png 150w, https:\/\/outliereditor.co.za\/wp-content\/uploads\/2021\/12\/Variants-of-Concern-in-Africa-ES-December-2021-768x768.png 768w, https:\/\/outliereditor.co.za\/wp-content\/uploads\/2021\/12\/Variants-of-Concern-in-Africa-ES-December-2021.png 1080w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/figure><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Disease surveillance<\/h2>\n\n\n\n<p>Genomic sequencing is not new to Africa and is already well established for other viruses. \u201cThe African continent is dealing with in excess of a hundred diseases per year,\u201d said Dr Alan Christoffels, the director of the South African National Bioinformatics Institute, one of four specialised genomics and bioinformatic centres in Africa.<\/p>\n\n\n\n<p>The other three centres are the African Center of Excellence for Genomics of Infectious Diseases in Nigeria, KRISP in South Africa, and the International Livestock Research Institute in Kenya, plus nine regional sequencing hubs from Morocco to the Democratic Republic of Congo (DRC) to Uganda.<\/p>\n\n\n\n<p>Each region is affected by different viruses. For instance, in South Africa, the focus has been on sequencing tuberculosis and HIV. DRC has been sequencing hemorrhagic fevers such as Ebola and monkeypox, the poliovirus and the plague. Uganda has been sequencing viruses such as Rift Valley Fever.<\/p>\n\n\n\n<p>The coronavirus pandemic has turned a spotlight on genomic surveillance and its crucial value in identifying variants quickly, tracking their transmission around the globe and helping governments to make quick public health decisions.<\/p>\n\n\n\n<p>In&nbsp;<a href=\"https:\/\/www.afro.who.int\/pt\/node\/14077\" target=\"_blank\" rel=\"noreferrer noopener\">September<\/a>&nbsp;2020, the 12 existing sequencing laboratories were launched by the WHO and&nbsp;<a href=\"https:\/\/africacdc.org\/news-item\/covid-19-genome-sequencing-laboratory-network-launches-in-africa\/\" target=\"_blank\" rel=\"noreferrer noopener\">Africa CDC<\/a>&nbsp;as a network to ramp up sequencing of the SARS-CoV-2 virus on the continent.<\/p>\n\n\n\n<p>\u201cThe aim of this network is to go beyond Covid-19,\u201d said Gumede-Moeletsi. \u201cMeaning that we are more than ready for any other pandemic that might hit our continent because we\u2019ve got well-established sequencing laboratory networks that are focusing on any emerging pathogen.\u201d<\/p>\n\n\n\n<p>The networks are based on three tiers, said de Oliveira of South-Africa\u2019s KRISP: specialised genomic centres (such as CERI-KRISP), regional genomics facilities, and national laboratories.<\/p>\n\n\n\n<p>\u201cAfrican countries should make efforts to be as active as possible at all three levels. In all, 32 countries in Africa now have genomic sequencing projects, according to the&nbsp;<a href=\"https:\/\/worldreport.nih.gov\/wrapp\/#\/search?searchId=6177bbfc2deece7278e9766f\" target=\"_blank\" rel=\"noreferrer noopener\">World Report<\/a>, a database of funded biomedical research across the globe started in 2012 by the US National Institute of Health.<\/p>\n\n\n\n<p>But there are gaps in capacity across the continent. For instance, a sample could be taken in Muyinga, Burundi, then it is sent 97 km to the capital, Gitega, before being delivered to the Uganda Viral Research Institute for testing.<\/p>\n\n\n\n<p>\u201cWe still have areas that when shipping the samples to wherever it is supposed to be tested is still taking a serious number of days before it reaches the laboratory itself. And, as such, we are not getting the report as soon as we were expecting,\u201d said Gumede-Moeletsi. The lag is not only affected by the time samples spend in transit to the lab, but also by the lack of reagents because of supply chain disruption, scarcity of skills, and \u201chesitancy of some health officials to release data\u201d, according to a&nbsp;<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/34672751\/\" target=\"_blank\" rel=\"noreferrer noopener\">research paper<\/a>&nbsp;in the journal Science.<\/p>\n\n\n\n<p>The technology is expensive.
